You are invited to participate in the data collection phase of the NSF-funded ARTIST project. We ask that you read this form and ask any questions you may have before agreeing to participate in the project.

This project is being directed by Joan Garfield and Robert delMas at the University of Minnesota, as well as Beth Chance at California Polytechnic State University.

Background Information:

The purpose of this project is to develop a Web-based assessment resource for first courses in statistics. Information on the project can be found at https:\\apps3.cehd.umn.edu\artist

Risks and Benefits of Being in the Study:

The study has no risks.
The benefit of participation is access to a variety of high quality assessment instruments and summaries of student performance information.


The records of this study will be kept private. In any sort of report we might publish, we will not include any information that will make it possible to identify you as a participant, or your students. Research records will be kept in a secure file; only researchers will have access to the records.

Voluntary Nature of the Study:

Your decision whether or not to participate will not affect your current or future relations with the principal investigators or their institutions. If you decide to participate, you are free to withdraw at any time without affecting those relationships.
